As a parent or legal guardian, I hereby give permission for my child to participate in PAL athletics and activities with the Police Athletic League of Kansas City, Inc., Board of Police Commissioners of Kansas City, Missouri and the Kansas City, Missouri Police Department.  I understand that participation in organized sports could involve some risk of personal injury or damage to the child, and I assume full responsibility for and all risk of damage or injury that may occur to the child while participating in the activities including, but not limited to transportation of the child to and from activities.  In consideration of the child being allowed to participate in the said activity, I fully release and discharge the Police Athletic League of Kansas City, Inc., the Board of Police Commissioners of Kansas City, Missouri and the Kansas City, Missouri Police Department and its members, agents, servants, and employees from any and all claims, demands, causes of action, present or future, whether known or unknown, anticipated or unanticipated and resulting from or arising out of, or incidents to my child’s participation in the activities.  I agree not to hold the Police Athletic League of Kansas City, Inc., the Board of Police Commissioners of Kansas City, Missouri and the Kansas City, Missouri Police Department, individual members, agents, servants and employees responsible for accidental injury or any loss or damage to property arising out of the activities.  I fully release the Police Athletic League of Kansas City, Inc., the Board of Police Commissioners of Kansas City, Missouri and the Kansas City, Missouri Police Department, individual members, agents, servants and employees for any and all obligation and responsibility to pay any hospital, medical, doctors and dental claims or any loss or damage of property that may arise.  I further agree to allow PAL personnel to authorize appropriate medical treatment if a parent or guardian is unavailable at the scene.  The undersigned also acknowledges that participation by said minor child in PAL Athletics/Activities is voluntary on the part of the said minor child and the undersigned. PAL Staff will also periodically track your child’s performance at school.  We will check your child’s grades and school attendance to insure the child is getting the most from his/her education.  By signing below, you authorize us to check this information.  All student information will be kept confidential. 
This form will be read out loud in front of a P.A.L. Officer! I _________________________, (the PAL Kid) and we ______________________, (the parents) enter into the following behavioral contract in order to create order and harmony at P.A.L. This contract will be reviewed when violated, at which time it is subject to either being continued or renegotiated. By our agreeing to the terms and conditions of this behavioral contract, all parties understand and accept that they are bound by the contract and are not free to vary from the terms and conditions. I ________________________, (the PAL Kid) agree that I will perform the behaviors listed below in the manner they are set forth and described.
  • I will not fight. (I will not be allowed back)
  • I will not bring real or fake drugs. I will not bring real or fake weapons. (Guns, Knifes) 
  • I will not play fight or touch others.
  • I will not back talk or use bad language—ever. 
  • I will not wear offensive clothes or do-rags.
  • I will not bring food or drinks onto the gym floor.
  • I will not steal, cheat or lie.
  • I will keep cell phones, mp3 players, video games, or any other electronic devices put away. 
  • I will pick up after myself.
  • I will be a team player when participating in activities.
  • I will have a good attitude and arrive (and remain) appropriately dressed in PAL approved attire.
  • I will keep my pants pulled up.
  • I will positively contribute and voluntarily help-out as needed.
  • I will use common sense and will seek an adult’s opinion if I have questions or concerns.
  • I will be respectful to ALL plus use good manners. (YES SIR, YES MA’AM, PLEASE, THANK YOU, ETC.)
(I _______________________, (the PAL Kid) agree that should I violate the contract by failing to perform the behaviors set out above that there will be consequences. As follows: First, I will talk with an adult (a PAL Officer) about what went wrong.  Additional problems will be addressed as follows:
  • I will be removed from others for a time out and my parent(s) will be informed. 
  • I will be sent home for one day.  
  • I will be sent home for one week.
  • If behavior is persistent or is severe: I understand I will not be allowed back.
P.A.L. Parent Section We, ________________________ (the parents), agree that if _________________________ (our child) performs the positive behaviors as outlined above (I wills), in addition to escaping any negative consequences outlined above, he/she will continue to earn the right to the following P.A.L. Programs including (but not limited to): Sports Field trips Games Free time Friends And most of all FUN!! Both parties acknowledge that this contract is entered into voluntarily and that the terms and conditions will be respected. If performance is accomplished, parents agree that the child shall not be denied privileges and other consequences. The child agrees that if he/she chooses the above negative behaviors (I will nots), it is not the parent(s) who have chosen to forfeit privileges but it was the child's decision--and, at this point, the child agrees no whining, complaining or rebelling will be tolerated. Do you understand the terms of this contract?
